SUMMARY
The Ace must be scored on an official Tournament governed by USGA rules. The ace must be witnessed by at least two other players and verified by a club official at the course played. The ace must be scored in a stipulated round (18 holes) or tournament format (scramble). ....

Participation & Prize Indemnification Agreement:

A. ELIGIBILITY

1. PARTICIPATION – The hole-in-one challenge is open to amateur golfers as defined by the United States Golf Association (USGA) 21 years or older, during official tournament play. A copy of the contest Participation & Prize Indemnification Agreement (“Agreement”) is available upon request.
2. PAYMENT – No purchase necessary.

B. DURING PLAY
1. GOLF COURSE CONTEST WITNESS – The participant is responsible for the presence of at least two (2) or more playing or non-playing witnesses, age 21 or older, capable of verifying the occurrence of a Hole-In-One. The witness(es) must be in full view of the green and tee, out of harms way, at the time the shot is taken. If a Hole-In-One occurs without the verifiable presence of a witness as stated above, or without proper ticket authorization as described herein, the Company shall have no obligation to pay the prize value.

2. ACCURACY OF CONTEST HOLE YARDAGE – From the point on the teeing area from which the shot is initiated to the flagstick, a qualified hole(s) must measure NO LESS THAN 135 YARDS. Actual yardage will be determined and verified by the golf course authorities at the golf course where the tournament is being played. This designated third party compliance resource from the point where the shot was taken to the point where the hole/pin was located. Do not trust the tee markers to be set at the correct yardage. If the actual yardage of a Hole-In-One is less than the yardage stated above, the Company shall have no obligation to pay the prize value. The tournament coordinator, participant and witnesses are responsible for tee and hole/pin reference points for third party validation. Club pro or course assistance is recommended.

3. NUMBER OF SHOTS – Only one shot per qualified hole defined as a “stroke” by the USGA Rules of Golf taken by an authorized participant is permitted. No practice shots, mulligans, or substituted shots for participant or other players are permitted.

4. MULTIPLE HOLES- IN-ONE – The Company agrees to pay the applicable prize value to a qualified participant for any hole-in-ones made during the course of the Pass coverage period. However, individual claims must be verified before the next round of coverage is valid.

C. AFTER PLAY
1. CLAIM NOTIFICATION – The participant must report the occurrence of a Hole-In-One to the Company’s claim department via telephone no later than the end of the first business day following the event. Claims must be reported to Jaure Winery Claim Department at 305-371-1847 or via email to: holeinone@jaurewinery.com

2. HOLE-IN-ONE EVIDENCE – The following documentation must be provided to the Company and its designated claim investigators in connection with a Hole-In-One claim: a) A signed and attested score card with two (2) participant, witness(es) (per B-1 above) and a signed affidavits from the golf course professional on duty at the time of the Hole-In-One. b) Verification that the participant making the Hole-in-One claim is of amateur status.

The Company may conduct a reasonable investigation to verify participant’s compliance, including requiring the participant, witness and/or playing partners to submit to a polygraphic examination.
